"I'm going to die," Astarion announces loudly. "And it's all your fault." He groans.

He's not going to die. Yes, he got targeted like hell in the last fight, but with you patching him up he'll be fine. He's being dramatic.

Astarion flings his hand to his forehead. "I'm going to wither away and die and it'll be because I have no blood left. Stop laughing!" And if he secretly likes being cared for by you, no matter how much you groan and he whines... well, some things stay secret, yes?

Background: From the game Baldur's Gate 3. He knows better than to bite the hand that feeds him; that doesn't mean he can't complain, though! He whines a lot and whatnot, but he does so because he knows he can and won't get hurt. He might not know it, but he trusts you.
